LASER TREATMENT FOR FINE WRINKLES, SUN DAMAGE OR ACNE SCARS In the past few years the use of various types of lasers has revolutionized facial rejunvenation and has added new dimensions for the treatment of acne and other scars. And other lasers can be used for treatment of facial capillaries and unsightly veins.
Lasers used for wrinkles, pigmentation and scars can be broadly divided into two categories; Ablative (hot) lasers and NonAblative (cold) lasers.
the Co-2 and Erbium lasers ablate (remove) the surface layers of the skin. The lasers heat the deeper, collagen-rich dermal layer and stimulate production of brand new collagen and elastic tissue. The surface layer is regenerated from reserve cells that survive the laser treatment, located in the depth of hair follicles and sebaceous glands. When all is said and done the old, sun-damaged, blotchy, wrinkled or scarred skin has been replaced to a greater or lesser extent, by brand-new skin. For many ofour patients, it feels like the age clock has been turned back by 10, 15 or 20 years. With incidentals, cost is around $3,000 for a full face, and much less for localized areas such as crow's feet or lips.
the CO-2 laser is by far more effective than the Erbium laser. But it also has a 30-50% longer recovery time. We own both of these lasers and use them as appropriate for the individual needs of each patient. Sometimes we even combine the two lasers for different areas of the face and upper neck. These laser procedures are usually performed in the office with local anesthesia. There is a one-two week initial recovery period, at the end of which the skin still looks quite red but can be covered with make-up, and with most if not all the wrinkles gone--looks quite good. But make-up is often needed for two-four months and a few months of post-laser treatment with various creams is generally needed as well. For patients with blotchy skin discoloration, at least several months of bleaching cream regimens and sunblocks are needed to help preserve a pristine appearance of the newly resurfaced skin.
Although results of such resufacing procedures can be quite spectacular, many patients simply cannot take the time off, nor are willing to put up with the after-care that may be needed. NON-ABLATIVE or COLD LASER treatment is the answer for them. Among the several systems currently available, Dr. Eremia prefers the COOL TOUCH II laser system. This highly sophisticated laser can heat the dermia and stimulate new collagen and elastic tissue formation without damage to the surface layer. While the results are not as immediate, and less spectacular, there is almost no down-time involved. A little redness lasting a few hours is generally it. A series of treatments, (we recommend six sessions, four to 12 weeks apart), is needed to achieve appreciable results. Costs range from $200-$300 per session based on the areas involved.
Dr. Eremia also practices as Associate Clinical Professor and Director of Cosmetic Surgery for the Division of Dermatology at UCLA.
